This is a patient referred with a history of previous Lasik who had cataract surgery by the referring surgeon with an Envista lens and now has both positive and negative dysphotopsia. There is an area of iris transillumination that is allowing light to hit the haptic optic junction on the IOL and this is causing scatter of light/positive dysphotopsia but the patient also has rather severe negative dysphotopsia suggesting he needs an exchange (he's also a bit hyperopic with this lens in the eye which provides another indication to exchange). The exchange is made more complex because the anterior capsule is fibrosed and the temporal haptic is really very well socked in and on top of that the pupil does not dilate as well as we would like to do this exchange. This video demonstrates some special techniques to free up this IOL and repair the iris defect.
